Key Responsibilities Deliver high-quality, patient-centered care to patients in their homes, including administering medications, changing wound dressings, and providing education on disease management and prevention Conduct comprehensive assessments of patients' physical, emotional, and social needs, and develop individualized care plans to address these needs Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure seamless coordination of care and optimal patient outcomes Provide emotional support and counseling to patients and their families, and connect them with community resources and services as needed Maintain accurate and timely documentation of patient care, including progress notes, medication lists, and treatment plans Participate in ongoing education and training to stay current with best practices and industry developments, and share knowledge and expertise with colleagues to promote a culture of excellence Qualifications and Requirements To be successful in this role, you will need to possess a strong foundation in nursing principles and practices, as well as excellent communication and interpersonal skills. You will also need to be compassionate, empathetic, and committed to delivering exceptional patient care.
