Job Highlights Company: CenterWell Home Health, a reputable home health care provider Location: Rome, GA, with remote work options available Start Date: Immediate openings available for the right candidate Compensation: Competitive salary and benefits package Position: Full-time Medical Social Worker for Home Health Services Job Description The Medical Social Worker will participate in the interdisciplinary care provided to home health patients, evaluating and developing personalized plans of care to address their emotional and social needs. The successful candidate will provide direction and supervision to Social Worker Assistants as required and work within the company's policies and procedures, applicable healthcare standards, governmental laws, and regulations. Key Responsibilities Assess the patient's social and emotional state, relating to their illness or injury, needs for care, and response to treatment Identify relationships between the patient's medical and nursing needs, financial resources, and available community resources Provide appropriate action to obtain available community resources to assist in resolving issues that may be impeding the patient's recovery Instruct patients and families in treating and coping with social and emotional responses connected with their condition Participate in the development and periodic re-evaluation of the physician's Plan of Care for the patient Observe, record, and report changes in patients' conditions and responses to treatment to the Clinical Manager and the interdisciplinary care team Participate in the discharge planning process and care coordination activities Supervise, instruct, and evaluate the performance of Social Work Assistants to ensure compliance with company, government, and professional standards Essential Qualifications To be considered for this role, you must have: A Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education Social Worker licensure in the state of practice, if required by state law or regulation A valid driver's license, auto insurance, and reliable transportation Proof of current CPR certification A minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice Knowledge of discharge planning needs, community resources, and the ability to obtain them Excellent oral and written communication and interpersonal skills The ability to read, write, and speak fluent English Preferred Qualifications While not essential, the following qualifications are preferred: Previous experience working in a home health setting Knowledge of medications and their correct administration Ability to organize tasks, develop action plans, set priorities, and function under stressful situations Flexibility in work hours and travel locally Ability to communicate effectively with patients, families, and all levels of the organization Skills and Competencies The successful candidate will possess the following skills and competencies: Strong assessment and planning skills Ability to work independently and as part of an interdisciplinary team Excellent communication and interpersonal skills Ability to prioritize tasks and manage time effectively Strong problem-solving and critical thinking skills Ability to maintain confidentiality and handle sensitive information Career Growth Opportunities and Learning Benefits At CenterWell Home Health, we are committed to the growth and development of our employees.
