Job Summary As an RN Case Manager, Home Health - Part Time/Days, you will be responsible for evaluating outcomes based on an integration of clinical, financial, and utilization data. You will utilize independent nursing judgment when integrating healthcare, working in structured and unstructured healthcare settings. Your expertise will be essential in promoting early intervention, enhancing patient/resident flow, and fostering interdisciplinary communication. You will work with clients across various age groups, from neonatal to geriatric patients, and must be knowledgeable about age-specific structured care methodologies. Key Responsibilities Integrate healthcare for clients from pre-admission to post-discharge, ensuring seamless coordination and sequencing of care Evaluate outcomes based on an integration of clinical, financial, and utilization data Utilize independent nursing judgment when integrating healthcare Promote early intervention, enhance patient/resident flow, and foster interdisciplinary communication Work with clients across various age groups, from neonatal to geriatric patients Document clearly and solve problems effectively Establish priorities, demonstrate strong flexibility and organizational skills, and work with little direction Facilitate groups and implement projects when necessary Be knowledgeable about reimbursement for services provided Essential Qualifications To be successful in this role, you will need: A Bachelor's degree in nursing preferred Graduation from a nationally accredited nursing program preferred Minimum of one to two years of professional nursing experience in a clinic care setting preferred In-depth knowledge of specialty area preferred Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possesses multistate licensure if in a Nurse Licensure Compact (NLC) state Obtains and subsequently maintains required department-specific competencies and certifications Depending on location, a valid driver's license may be required Preferred Qualifications While not essential, the following qualifications are highly desirable: Experience working in home health care Knowledge of electronic medical records and care management software Strong communication and interpersonal skills Ability to work independently and as part of a team Flexibility and adaptability in a fast-paced environment Skills and Competencies To excel in this role, you will need to possess: Strong nursing skills and knowledge Excellent communication and interpersonal skills Ability to work independently and as part of a team Strong problem-solving and critical thinking skills Ability to prioritize tasks and manage time effectively Knowledge of age-specific structured care methodologies Ability to adapt to changing situations and priorities Career Growth Opportunities and Learning Benefits At the Good Samaritan Society, we are committed to helping our employees grow and develop in their careers.
