Key Responsibilities: Conduct comprehensive home health assessments on pediatric patients, from newborns to teenagers Develop and implement individualized care plans, in collaboration with the patient's family and healthcare team Provide skilled nursing care, including medication management, wound care, and treatment of various medical conditions Educate patients and families on self-care techniques, disease management, and prevention strategies Monitor patient compliance and adjust care plans as needed Perform various treatments, such as NG or GTube feedings and ostomy care Maintain accurate and detailed records of patient care, using electronic charting tools Collaborate with the patient's healthcare team to ensure seamless care coordination Requirements: Current and unencumbered state licensure as a Registered Nurse Minimum 1 year of recent clinical nursing practice, preferably in ICU or pediatrics Demonstrated passion for working in pediatrics and providing high-quality care to children Experience with NG or GTube feedings and ostomy care is preferred Prior home health experience is required Strong communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams What We Offer: Competitive salary, ranging from $57,000 to $79,000 per year Flexible scheduling, with full-time and part-time positions available Benefit eligibility begins at 20+ visits per week, including: Medical, Dental, and Vision coverage Company-paid life insurance Employee Assistance Program Fidelity 401K plan PTO and paid time off Attractive visit rates and mileage reimbursement Company-issued iPad and electronic charting using Kantime Education and clinical training opportunities, both classroom and virtual Career development and advancement opportunities Free CEU access and $500 referral bonuses for teammates who refer a nurse Our Culture: At Thrive Skilled Pediatric Care, we pride ourselves on a culture of excellence, respect, integrity, compassion, and social responsibility.
